ASSESSMENT
Our Occupational Therapists carry out assessments of Clients Occupational Functioning and Mobility . Our Occupational Therapists are approved to carry out specialist diagnostic tests including Cognitive, perceptual and memory batteries (where required) and Complex Moving and Handling assessments. All Occupational Therapy Associate Staff are Fully DBS Checked, HCPC and Royal college of Occupational Therapist Registered. All Staff are fully trained to UK Standards and are covered for professional indemnity insurance by Baylens insurance LTD .
At your first visit, the Occupational Therapist will conduct a thorough initial assessment of the current difficulties and advise whether further appointments are required to complete a specialist assessment or whether a treatment can be commenced. In some instances, following the assessment, for example where ADHD or Autism are being considered, additional specialised assessments may be recommended. It may also be helpful to contact the Clients School or workplace to gather more information to help complete the assessment
By the end of the end of the assessment, the Occupational Therapist will discuss the formulation, diagnosis where relevant and a management plan including treatment options. Following the full assessment, a report will be provided within1 month of the final assessment appointment.
Occupational Therapists general appointments lasts up to 90 minutes. Occupational Therapist follow up appointments last up to 25-45 minutes.

TREATMENT
Your treatment package will be tailored to your individual needs. This may include Occupational Therapist reviews, physical and psychological therapies for individuals, Equipment and adaptation recommendations or recommended referral to other professionals. Our Occupational Therapist work closely with Physiotherapists, GPs, Equipment suppliers, Nursing staff including specialist nurses, speech and language therapists, dieticians and other professionals within the Health and social care Sector.
If therapy is indicated, the most relevant type of therapy will be planned either individually the Client or with the family as a whole, or a combination of both. We have a variety of highly experienced therapists based within the service and close links with external industry specialists and clinicians in the following professions (Physiotherapy, General Practice, Speech and language Therapy and Community Nursing, Handyperson service, Equipment Prescription)
Sometimes medication is appropriate and the Occupational Therapist may recommend it. However it is important to note the Occupational Therapist is not a medical prescriber. OT 4 Life recommend consulting with your GP regarding any medication needs Prior to commencing medication. Should medication be highlighted at the time of the assessment, the Occupational Therapist is unable to give clear recommendations of prescriptions for treatment and medications however may be able to share previous clinical experiences of medication or medical intervention and the effects observed in previous anonymised cases. The final decision however must be made with the client and Medical Practitioners.
PAYMENT
Booking fees are to be paid at the time of appointment booking. The Booking Fee is 40% of the overall cost for initial assessment and travel costs. The Occupational Therapist consultation fees include an initial assessment report. However additional reports will incur an additional fee. Other miscellaneous administration services such as filling forms, telephone/email correspondence, will incur a fee. The fee for follow up and administration work is set at an hourly rate and we ask that you read this prior to signing the consent form.
Please note that even if someone else is paying your bill you are responsible for paying any charges, if they do not pay. If you have medical insurance you will be required to pre-pay for your appointment and be given a receipt which you can submit to your insurer for reimbursement. We advise that you check with your insurer that they will reimburse you, prior to booking.
All new patient consultation charges are inclusive non-refundable deposit at the time of booking. Completed appointments and reports are non-refundable. Payment is due at the time of booking to secure the session and final payment for the remaining costs are due 3 working days post assessment date. Late payment will incur a £50 additional charge per week of overdue payment. OT4 Life reserve the right to withhold final report until full payment is made and any works or further treatments planned will be placed on hold until payment is received .
CANCELLATIONS AND NON-ATTENDANCE
Unfortunately, we are not able to see all new patients as promptly as we would like to due to the large numbers of referrals. If you are unable to make your appointment, please provide at least 7 working days notice for new patient assessment appointments. If the Appointment is cancelled within 2 working days 50% of the initial assessment fee will be charged. If the appointment is cancelled within 24 hours of the assessment full payment is due for the assessment.
MEDICAL RECORDS
We are required to keep complete medical records, which includes personal and sensitive data. The medical records are electronic and encrypted. Any paper records including the initial consent forms, letters outside medical records, will be scanned and disposed of appropriately. The records are kept in an encrypted cloud service (which is GDPR compliant) and in an electronic medical records system (Writeupp) which is GDPR compliant.
Upon Completed initial assessment you will be provided with a copy of your initial assessment report as part of the service provided. You are entitled to review your medical record and clinical entries regarding follow up work at any time, unless in the very rare circumstance that your doctor/therapist feels that by viewing your records, your emotional or physical well-being will be jeopardised. If you wish to view your records, we recommend that we review them together to minimise any confusion or misinterpretation of medical terms. Printing and postage may incur a small fee. Information will be provided to you within 30 days of agreed release.
Your medical records are kept for the time necessary to provide the service requested, however outside of this we will hold your records for a period of 20 years following the end of the assessment and/treatment to comply with the recommendations of the Information Governance Alliance with regards to mental health records. After this date, all data will be securely deleted unless requested otherwise.
